We all use Facebook to connect socially and LinkedIn to be known for work. But what defines us to those that care for us? What provides data to support person centred care?

RemindMecare (aka ReMe) is the first to address this and in a manner that is not just an app but clinically relevant. So it’s usable from diagnosis to end of life, by everyone and every care business.

ReMe makes it possible to capture a life digitally; from photos to videos, music and memories. We call this Electronic Life Records (ELR). It’s not care planning data, its personal data.

Through easy uploading and bespoke activities, a ReMe My Story grows to become an invaluable resource of personal knowledge, that’s available to anyone in the care circle.

ReMe provides family, friends and carers with rewarding bespoke activities that entertain whilst building a vault of personal information. Together you explore pictures, music and video collecting favourites along the way. Fun to use, it’s a safe place to store and share information that really matters to the person cared for.

We built ReMe after witnessing our young family members unable to connect with relatives, and carers treat our loved ones as numbers as they had no knowledge of the person. Since then, we’ve worked with people living with dementia, learning disabilities and care services to develop a rewarding means to build understanding between different generations, cultures and backgrounds, always keeping the person cared for and their loved ones in control of their personal information.

When moving to a new care setting, ReMe’s there to introduce deep knowledge of the person to new carers. ReMe presents the person rather than a list of symptoms. ReMe’s used in care homes, day care centres and memory clinics and is used in Kingston Hospital where they’re evaluating how knowledge of the individual can reduce medication and the length of stay in acute wards (google Kingston Hospital, RemindMecare for info.).

If you choose to upgrade to ReMe’s premium services you can collaborate with family and carers using shared calendars to provide reminders and keep everyone up to date. You can even use a smart speaker like Amazon’s Alexa or Google Home to engage the individual when they’re alone using ReMe’s knowledge to entertain and delight, as well as provide medication, hydration and scheduling alerts.
